Procedure to fill the form
First download the form, get it printed fill for the practice than submit online now another
form will appear with application no. and filled data. Get it Printed and send this form
along with required documents and pay order / bank draft of Rs. 2500/- in favour of
Jinnah Sindh Medical University, Karachi to Admission cell (Admission Cell, Jinnah
Sindh Medical University, Rafiqui H.J Shaheed Road, Postal Code # 75510, Karachi)

Ans.: DOCUMENTS REQUIRED WITH THE ADMISSION FORM
1. Candidates must attach the following documents with the Admission Form,
the documents attached with the Admission Form must be attested by a Government
Officer in BS 17 or above. The stamp of the officer must bear his / her full designation
and current place of duty:
(i) Two (02) attested copies of Matriculation (SSC) Certificate or result card issued by a
Board of Intermediate and Secondary Education in Pakistan. Computer generated result
cards will not be acceptable. Candidate having foreign qualifications (O-Level, 10th
Grade and equivalent) must provide the attested copies of Equivalence Certificate issued
by IBCC. A valid Interim or Provisional equivalence certificate issued by IBCC will also
be acceptable.
(ii) Two (02) attested copies of HSSC Pre-Medical result card issued by a Board of
Intermediate and Secondary Education in Pakistan. Candidates having foreign
qualifications (e.g. A-Level, 12th Grade and equivalent) must provide the attested copies
of Equivalence Certificate issued by IBCC. Valid Interim or Provisional certificate by
IBCC will also be acceptable.
(iii) Two (02) copies each of computer generated result cards of MDCAT/SAT II/MCAT
scores (Not to be attested).
(iv) Two (2) attested copies of Domicile Certificate of the candidate (In case of Children
of Overseas Pakistanis/Dual Nationality Holders category only, domicile of
Father/Mother shall also be acceptable). No other certificate or document (e.g. Birth
Certificate, B-Form, CNIC etc.) is acceptable in lieu of Domicile Certificate of the
candidate. Any candidate found to have domicile of more than one place shall be
disqualified.
(v) Two (2) attested copies of CNIC of Father / Mother. Overseas Pakistanis/Dual
Nationality Holders may provide attested copies of their Pakistani Passport/ NIC/
NICOP/ POC or any other official document providing their Pakistani nationality.
(vi) Two (02) recent photographs. One to be pasted on the Admission Form and to be
attested on the front. Passport size.
(vii) Original Fitness Certificate issued by a Registered Medical Practitioner /
Government Medical Officer. (Optional)
(viii) Original Affidavit on minimum Rs.50/- stamp paper as per specimen given
(unde
rtaking)
(ix) Candidates claiming to be Muslim Haffaz-e-Quran shall attach Two (2) attested
copies of Hifz-e-Quran Certificate issued by a registered Madrsa.
Q. 3 I have domicile of Balochistan, can I give the marks of NUMS test?
Ans: NUMS test is only for Regions e.g., Islamabad, Azad Kashmir, Gilgit, Hunza, and if you are domiciled of any province in that case result of provincial admission test will be counted for calculating merit.
Q. 4 IBCC has not yet issued me equivalence certificate for any reason, can you accept my certificate as such for admission purposes?
Ans : No, we can’t accept your foreign qualifications as such without equivalence certificate
issued by IBCC. You must settle your issues with IBCC before you apply for admission.
Q. 5 Shall I have to apply separately for admission to these colleges?
Ans: No, there is centralized admission system for government / private sector medical/dental
institutions of the Sindh province. JSMU will select the candidates and place them in deferent
colleges on the basis of their merit and choice. The candidates give their options/preference for various colleges on one single admission form.
Q. 6 Should I submit Insurance certificate along with form
Ans; Only wealth statement and under taking to be submitted along with form
Insurance is to be submitted to the Institution in which you get admission
Note: those they have already submitted the application form they can now submit wealth
statement as a short documents with clearly mentioning Name, Fathers name, CNIC no. and
Application no.
